Orthodontics

During fixed orthodontic treatment, a bracket coming off, the end of the wire poking or a tie loosening are common situations. These do not mean that the treatment has failed; they are hitches that can be expected during the process. What matters is knowing what to do when you say “my bracket came off” and going to the clinic with the right timing. As DijiDent Oral and Dental Health Polyclinic, in Türkiye we give our patients a practical roadmap for these situations from the start.

When a Bracket Comes Off

Even if the bracket separates from the tooth, it usually remains attached to the wire and does not fall out of the mouth. The steps to follow are:

  • Do not panic: This is not a medical emergency.
  • If the bracket is attached to the wire, leave it in place: Do not try to remove it.
  • If it has come off completely, keep it: Put it in a small box and bring it to your appointment.
  • If it causes discomfort, use orthodontic wax: It covers the area and prevents cheek irritation.
  • Call your clinic: If your appointment is soon, it can wait; if it is far off, an interim appointment is planned.

A detached bracket remaining unbonded for a while causes the movement of that tooth to pause. For this reason, waiting a long time may delay treatment. The general approach is determined by how many days are left until your next appointment.

If the Wire Is Poking

As the teeth move, the back end of the wire may lengthen and irritate the cheek. In this case, first apply orthodontic wax; softening the wax between your fingers and pressing it onto the poking end is enough. If there is no wax, a piece of clean cotton can be used temporarily.

Do not try to cut the wire itself with scissors or nail clippers. The cut piece may be swallowed or get into the airway; in addition, when the integrity of the wire is disrupted, the planned movement is affected. If a sore has formed, rinsing with lukewarm salt water may be soothing.

If a Tie or Elastic Comes Off

The small elastics that tie the brackets to the wire may loosen or break over time. This is usually not urgent; they are renewed at the next appointment. However, if more than one tie has come off, the likelihood of the wire coming out of place increases; in this case, inform your clinic.

Why Do They Come Off?

  • Biting hard foods: Apples, carrots, corn, hard bread crusts and nuts are the most common causes.
  • Sticky foods: Turkish delight, caramel and gum can pull the bracket.
  • Biting with the front teeth: Biting large bites without cutting them up.
  • Bite contact: Especially in a deep bite, the lower brackets may make contact with the upper teeth.
  • Habits: Biting pens, nail biting, chewing ice.
  • Impact: Trauma from sports or accidents.

If brackets come off frequently, the cause should be investigated. Eating habits are reviewed; if there is a bite-related contact, your dentist can adjust it.

How Much Does It Affect Treatment?

A single bracket coming off usually causes a small delay. However, repeated detachments can significantly lengthen the total duration. Each detachment means that the movement of that tooth pauses and sometimes reverses. For this reason, following the eating rules is not just a recommendation but a factor that directly affects the duration of treatment.

Band Loosening

Metal bands placed on the back molars may also loosen over time. When a band loosens, food accumulates under it and the risk of caries in that area increases; in addition, control of tooth movement decreases. If you notice that the band is moving or you feel a bad taste in that area, inform your clinic. This is a situation that is less noticed but more neglected than a bracket coming off.

What You Should Keep With You

We recommend that our patients undergoing orthodontic treatment prepare a small kit: orthodontic wax, a small mirror, an orthodontic toothbrush, an interdental brush and the clinic’s phone number. This kit makes things easier, especially when traveling.

If You Are Out of Town or Abroad

If you are planning a long trip, inform your dentist in advance. Your treatment stage can be planned accordingly or a check-up appointment can be given before the trip. In an emergency, you may need to see an orthodontist where you are; in this case, having your treatment records with you makes the process easier.
