Digital Smile Design

In smile aesthetics, the teeth are only one part of the picture; the lips that frame them are at least as decisive as the teeth. Even if two people have the same tooth form, their smiles look completely different because their lip movements differ. The smile line is the basic concept that defines this relationship and forms the starting point of aesthetic planning. As DijiDent Oral and Dental Health Polyclinic, in Türkiye we build the design by evaluating this frame before the teeth.

What Is the Smile Line?

The smile line refers to the relationship that the lower edge of the upper lip forms with the teeth when smiling. Three basic types are described. In a high smile line, all of the teeth and some gum above them are visible. In an average smile line, the teeth are largely visible, and the gum appears only between the teeth. In a low smile line, part of the teeth is covered by the lip.

None of these is a “problem” on its own. However, this type directly determines which treatment can be applied and what the aesthetic goal should be.

How Does It Affect Planning?

In a patient with a high smile line, gum margins and crown edges are much more visible; for this reason, harmony in these areas becomes critical. In a low smile line, the incisal edges of the teeth come to the fore and details in the gum region are less noticeable. The same treatment requires different levels of precision in two patients.

What Is a Gummy Smile?

Showing a noticeable amount of gum when smiling is popularly called a gummy smile. This is not a disease; it is a matter of aesthetic perception and does not require treatment as long as it is not found bothersome. However, if the person is bothered by it, the cause must be determined first.

  • Teeth appearing short: The gum may be covering part of the tooth; the tooth itself is not short.
  • Hypermobile upper lip: The lip rises more than expected when smiling.
  • Upper jaw structure: Conditions related to the vertical development of the jaw.
  • Gum overgrowth: Tissue increase due to certain medications or inflammation.
  • Wear: The proportion changing as a result of the teeth becoming shorter.

If the cause is different, the solution is different. For this reason, no single standard treatment can be spoken of; a detailed evaluation is needed.

Approach Options

Adjustment of the gum level, orthodontic approaches, restorative solutions or a combination of these methods may come into consideration. In some cases, approaches aimed at lip movement may be considered. In advanced skeletal conditions, surgical support may be needed. Which option is suitable depends on correctly identifying the cause, and the decision is made after the examination.

The Amount of Tooth Visible at Rest

The amount of tooth visible below the upper lip when the mouth is closed and the face is at rest is also taken into account in planning. This value varies with age and is an important reference in determining tooth length. An evaluation made only while smiling means basing it on a moment that does not cover most of daily life.

Appearance That Changes with Age

The smile line is not fixed. With age, the mobility of the upper lip tends to decrease; for this reason, the amount of upper teeth visible when smiling decreases over time, while the lower teeth become more visible. This natural change is taken into account in aesthetic planning. A tooth length suitable for a young patient is evaluated differently at an older age.

The Role of Photo and Video Recording

This evaluation cannot be made soundly with a smile the patient forces while sitting in the chair. To capture the natural smile, a video recording is taken and lip movement during speech is observed. For this reason, at our clinic we apply video recording as a standard step in aesthetic planning.
