Implant Treatment

In classic implant treatment, after the implant is placed, a few months are waited for it to fuse with the bone, and a tooth is made on it after this period. Especially for patients who have lost a front tooth or who are going to lose all their teeth, this waiting period can be challenging aesthetically and socially. In suitable cases, a temporary tooth can be attached on the implant shortly after it is placed. This approach is called an immediate loading implant. As DijiDent Oral and Dental Health Polyclinic, we offer this option to our patients in Türkiye while clearly stating who it is suitable for and its limits.

What Is Immediate Loading?

Immediate loading is the attachment of a temporary tooth or prosthesis on the implant on the day it is placed or within a few days. This temporary tooth offers an aesthetic and functional solution while the bone fusion process continues. After fusion is complete, the temporary tooth is replaced with the permanent superstructure.

Understanding the Concepts Correctly

  • Immediate placement: Placing the implant in the extraction socket on the day the tooth is extracted, in the same session.
  • Immediate loading: Attaching a temporary tooth on the implant within a short time.
  • Conventional loading: Making a tooth on the implant after fusion with the bone is complete.

These two approaches can be applied together or separately. For example, a tooth can be extracted and an implant placed on the same day with a temporary tooth attached immediately; or an implant can be placed in a healed area and a temporary tooth made on it immediately.

What Conditions Are Required for Immediate Loading?

Sufficient Primary Stability

The most important condition is that the implant seats tightly enough in the bone at the moment it is placed. How stable the implant is is measured during surgery. If sufficient stability cannot be achieved, the conventional waiting period is preferred so that the implant can fuse safely. For this reason, the decision for immediate loading is often made definitively during surgery.

Suitable Bone Quality and Volume

Bone density and volume must be sufficient. Immediate loading is generally not suitable in cases requiring extensive bone grafting.

Absence of Infection

If there is an active and widespread infection in the extraction area, it may be necessary to wait for healing first.

Bite and Load Control

The temporary tooth is designed so that it will not be exposed to chewing forces during the fusion period. In single-tooth cases, the temporary tooth is prepared so that it does not contact the opposing tooth. In full-arch cases, the implants are connected to each other with the temporary prosthesis so that forces are distributed evenly.

Who Might It Be Suitable For?

  • Single tooth loss in the front region: Patients for whom not being left without a tooth is aesthetically important.
  • Full-arch treatment: A same-day temporary fixed prosthesis on a suitable number of implants for patients who will have all their teeth extracted or who are edentulous.
  • Short visits: Allowing patients coming from abroad to return home from the first visit with temporary teeth.

Who Requires Careful Evaluation?

  • Heavy clenching and grinding: Excessive forces can negatively affect fusion.
  • Low bone density: Sufficient primary stability may not be achieved.
  • Uncontrolled diabetes and heavy smoking: Healing risks increase.
  • Need for extensive grafting: Loading the implant before the graft matures is not recommended.

Advantages

  • Not being left without teeth: It provides important comfort aesthetically and socially.
  • Fewer visits: In some cases, the total number of treatment visits may decrease.
  • Gum shaping: The temporary tooth can help the gum heal in a way that suits the permanent tooth.
  • Psychological comfort: Especially for full-arch patients, transition to fixed teeth without going through a removable denture period.

Limits and Things to Pay Attention To

Immediate loading is not suitable for every case. Although successful results are reported in suitable patients, the patient’s compliance with the recommendations during the healing period is of decisive importance. It is necessary not to bite hard foods with the temporary tooth, to follow the soft diet your dentist recommends and to attend check-ups regularly. When loosening or a fracture is noticed in the temporary tooth, the clinic should be contacted without losing time.

Transition from the Temporary Tooth to the Permanent Tooth

After fusion with the bone is complete, the temporary tooth is removed and a digital impression is taken to prepare the permanent superstructure. The gum shaped during the temporary tooth period can contribute to the permanent tooth having a more natural appearance. During this transition, your experiences regarding the position, color and form of the temporary tooth are also taken into account in the design of the permanent tooth.

The Role of Digital Planning

At DijiDent, in cases where immediate loading is planned, we design the implant positions and the temporary prosthesis in advance with tomography and intraoral scan data. In suitable cases, temporary teeth can be prepared before surgery and transferred to the planned position with a surgical guide. This approach aims to increase the predictability of same-day treatment. However, the final decision is made according to the implant stability measured during surgery; we share this possibility openly with our patients before treatment.

Frequently Asked Questions

Successful results are reported in appropriately selected cases when recommendations are followed. Suitability is evaluated by the dentist.

Soft foods can be eaten; however, you need to avoid biting hard foods with the temporary tooth.

Preliminary planning is done; however, the definitive decision is made during surgery by measuring the stability of the implant.

After fusion with the bone is complete, the permanent superstructure is usually made within a few months.
