Orthodontics

The first question everyone asks before starting orthodontic treatment is the duration. On the internet, exact figures are often given; yet this duration can differ significantly even between two patients of the same age. Orthodontic treatment duration consists of a combination of more than one factor, and some of these are directly in the patient’s hands. As DijiDent Oral and Dental Health Polyclinic, in Türkiye we prefer to share the duration as a realistic range.

Factors That Determine the Duration

The Complexity of the Case

Relieving mild crowding and correcting a malocclusion are not completed in the same time. The type of movement to be made is decisive: tipping a tooth takes less time than moving its root. Rotation corrections and vertical movements require more time.

Skeletal Component

If the problem is not only in the teeth but in the jaw relationship, the plan expands. In children, development can be guided; in adults, surgically assisted plans may come into play and the duration is longer.

Tooth Extraction

In extraction plans, closing the resulting gaps in a controlled manner takes time. This is included in the plan from the start.

Age

During growth, the bone response is usually faster. In adults, movement may be somewhat slower.

Biological Response

The rate of bone remodeling differs from person to person. The same force may give results at different speeds in two patients. This is a variable that cannot be controlled.

Factors in the Patient’s Hands

A significant proportion of the causes that prolong the duration arise during treatment and can be prevented:

  • Missing check-up appointments: Each delay means postponing the movement planned for that period.
  • Repeated bracket detachment: The movement of the relevant tooth pauses, sometimes regresses.
  • Not wearing clear aligners enough: This is the most common cause of delay.
  • Neglecting the use of elastics (rubber bands): They play a critical role in bite corrections.
  • Neglecting oral care: When gum problems develop, treatment may be paused.
  • Caries development: When interim treatments are needed, the plan pauses.

The Importance of Elastics

This is the element patients most often underestimate. Elastics used to correct the position of the jaws relative to each other work depending on daily wearing time. Elastics that are only worn at night or often forgotten can prolong this stage of treatment by months. Elastic use is not a suggestion but a working part of the plan.

Acceleration Methods

Various approaches aimed at accelerating tooth movement have been developed. These include some surgically assisted techniques and device applications. These methods can be considered in certain cases; however, they are not suitable for every patient and the scientific debate about their effects continues.

It is necessary to be clear here: tooth movement is a biological process and cannot be accelerated without limit. Applying excessive force does not speed up movement; it can damage the root structure and increase pain. We recommend that you approach with caution approaches that promise unusually short durations.

Finishing by a Certain Date

Our recommendation to patients planning treatment before a wedding or an important event is clear: plan backwards and leave buffer time. In addition, in some cases the goal can be staged as prioritizing the arrangement of the visible area rather than a full correction. Discussing this expectation from the start prevents later disappointment.

The Finishing Stage at the End of Treatment

Most patients expect treatment to end when the teeth look straight. Yet in the final stage, fine-tuning of bite contacts, adjustment of root positions and completion of midline harmony are carried out. Although this finishing stage does not create a visible change, it determines the stability of the result. For this reason, we approach the request “they’re already straight, let’s remove them now” with caution.

Tracking Progress

When treatment takes a long time, motivation may drop. Taking digital scan records at certain intervals and comparing them with the starting point allows you to see progress concretely. Changes that are not noticed in the mirror appear clearly in this comparison.

Frequently Asked Questions

Treatment can usually be continued; however, you need to inform your dentist and the plan needs to be reviewed.

It varies significantly according to the case; the exact duration is shared after the evaluation.

Biological response, missed check-ups or detachments may be factors; your dentist will explain the reason.

A general claim of speed superiority is not correct; the duration depends on the case and compliance.

Yes. The planned movement is postponed and the total duration is prolonged.

Finishing before the goal is reached may lead to the result not being stable.

Your duration is specific to your case.
